Hey Dan,

I don't think this can be stated enough.

Once you spot the fish, and start the cast, you already know where your want to place the fly!

If the focus is the fish and the fly, there seems to be a tendency to direct the FLY to the fish, which drops the rod tip, toward the fish, and opens the loop. Having a focus on the loop, instead of the fly, is the key for me. This focus away from the fly/fish helps calm my nerves!
I have found this is true for permit and bonefish, too.

I think this is what makes sight fishing such a challenge, and if the quarry is 5 ft. long, it takes a LOT of discipline to focus away from the fish and fly.
